Mandatory and nationwide, it seeks to unify criteria and modernize occupational medical evaluation processes, ensuring that employers, workers, contractors, private health insurance providers (IPS), and occupational risk administrators operate under clear and consistent rules. Its purpose is to guarantee safer and healthier work environments.

One of its key contributions is clearly establishing who is eligible and the responsibilities of each actor. Employers must assume all costs of evaluations and additional tests, including transportation and meals if required. They are also required to schedule exams during working hours, provide medical reports, and adjust working conditions according to recommendations, within a maximum period of 20 business days. The ARLs, for their part, have the obligation to provide technical assistance and training, although they do not cover exams. IPSs and occupational physicians must guarantee the quality, confidentiality, and safeguarding of clinical information. Workers, in turn, must attend punctually, provide accurate information, and participate in self-care activities such as active breaks.

Types of evaluations

The resolution also redefines the types of evaluations: entry, periodic, exit, return to work, and follow-up, with frequency based on the risk level. It also introduces the requirement to have detailed job profiles—previously known as job descriptions—that include functions, risks, and job conditions as a basis for the Occupational Health and Safety Management System.

Another key element is the occupational medical record, which must include minimum content, be kept for at least twenty years, and be kept strictly confidential. The employer cannot keep copies; only the physician and the IPS have that authority.

Specific tests

The resolution protects fundamental rights by prohibiting pregnancy, HIV, or serology tests from being required as a general condition of admission or stay. They are only possible with informed consent and in cases of documented risk. Regarding alcohol or drug use, testing is only permitted for high-risk activities or activities involving third-party liability, always within a preventive program documented in the SG-SST, with quality controls and clear protocols.

Finally, the occupational health assessment must include a diagnosis, restrictions, recommendations, and an assessment of physical and mental fitness. When a pathology is suspected, the worker must be promptly referred to the health system.

Conclusion

Resolution 1843 of 2025 is consolidated as a robust, modern framework focused on the comprehensive health of people. By distributing responsibilities, protecting rights, and updating practices, it strengthens the culture of prevention and promotes more dignified and healthy work environments for everyone in Colombia.

La Resolución 1843 de 2025, expedida por el Ministerio del Trabajo, marca un avance importante en la gestión de la salud laboral en Colombia.
